The hospice medical director provides oversight of patient care and support to the hospice care team. They attend team conferences to discuss the plan of care by assisting in establishing goals, and participating in decisions regarding patient care.
Our physician consults with the primary physician in caring for patients and visits patients wherever they reside to care for their medical needs.
The registered nurse case manager coordinates the plan of care, based on the patient goals, with the hospice medical director through the initial and ongoing nursing assessments. The nurse visits the patient weekly and/or depending on the level of care needed to ensure all symptoms are effectively managed and that the patient and family needs are being met.
The social worker provides initial and ongoing psychosocial assessments (mental, emotional, social, and spiritual dimensions of the patient’s care) to establish a psychosocial plan of care. While providing families and patients with hospice information this could include helping the patient with a Do Not Resuscitate (DNR) order, assisting with finding community resources, and making arrangements for nursing home placement or transfer to inpatient facility.

Certified Nursing Assistants help with bathing, personal care, and light housekeeping based upon the patient’s goals and plan of care.
